Quantum will evaluate the MDF request and send a notice of pre-approval, denial, or a request for more information.
The partner uses the MDF request form to communicate successful execution of the pre-approved plan, attaches supporting material, and requests payment of MDF funds.
Quantum evaluates the claim for validity and sends a notice of approval for payment, denial, or a request for more information.
Quantum funds post-approved claims for the amount specified in the plan.
